Now that you know how to find Billing Support Questionnaire (BSQ) Errors (see this article linked here), how can they be fixed?
Fixing a BSQ Error
First, Users can navigate to the month of the questionnaire on the member's Billing tab. Click on Action and Cancel Update to Questionnaire:
Clicking on this option instead of Edit Questionnaire will bring the BSQ back to a Potential billing state, which helps Users to "start fresh."
Now Users can click on Actions again and choose Add Questionnaire to move forward with fixing the BSQ error.
Frequent BSQ Errors
AH+ Errors
There is a question in the BSQ that asks if a member qualifies for Adult Home Plus (AH+) Care Coordination services:
This question is asking about the member's eligibility for this program. There is a list that the DOH has on file for all members who qualify for these services. If a member is on the list and the question is answered with a "No," the BSQ will reflect this error below:
To fix this, change the answer from "No" to "Unknown" for this question if your agency is not providing these services to your member, but the member is on the DOH list.
The reverse is also true. If the answer to the AH+ question is "Unknown" but the member is not on the DOH list, the below error will pop up:
To fix this, change the answer from "Unknown" to "No" on the BSQ. Once the changes are made, the BSQ should sync up successfully to MAPP.
No BI for (CIN Number) as of (DOS)
This error may come up on a BSQ that is for a segment that is pended.
If a member is pended due to a reason outside of Diligent Search, then the member's segment is not billable and MAPP does not provide a Billing Instance (BI).
To check the pend reason, go to the member's segment and click on details.
In the details of the segment, you will see the pend reason. If the pend reason is not due to Diligent Search, then this BSQ will not sync, which is valid. No further action is needed to be done by the User.
Cannot select HCBS only and another
This BSQ error will come up when you choose the option Children's HCBS only and another pre-condition.
Within the questionnaire, if the member's pre-conditions include Children's HSBC and other conditions, choose this box below and also include what those conditions are.
It will only be appropriate to choose Children's HCBS only if no other pre-condition exists.
Duplicate Billing Instance
These errors usually occur when there is an internal processing error on FCM's end. The error does not cause any syncing problems but can look concerning to users when they see an error next to their BSQ. If users see this "error" but their BSQ shows as Added (synced), know that this means that it has reported to MAPP.
FCM is able to manually fix these so that the error notifications are no longer next to the BSQs, just send us an email!
Invalid Princ Prov Code for serv
This error means that the member has some sort of Medicaid restriction that surfaced for the month that a User is attempting to bill. This restriction is preventing the BSQ from reporting to MAPP.
Typically the RE Codes that are found to be causing this error are "AL" or "NH." Users can hover or click on the RE Code on the member Overview to view what these codes mean.
FCM cannot override this error. The User must work with the member's Medicaid to address the code; if the code is valid, then the User will not be able to move forward with billing for that month.
1 SQC/2 or More Chronic Conditions Reqd
Based on a MAPP Release in December 2022
This error means that either 1 Single Qualifying Condition should be checked off or at least 2 chronic conditions are required to be checked off on the member’s BSQ.
- Single Qualifying Conditions:
- HIV/AIDS
- Serious Mental Illness/ Serious Emotional Disturbance
- Sickle Cell Anemia
- Complex Trauma (under 21 years of age)
A common example of when this error is seen is when a member has only Mental Health checked off as their condition in the BSQ and no other conditions marked.
HH Qual Condition Description required
Based on a MAPP Release in December 2022
This error means that a description of the member's qualifying condition is required if a member's qualifying condition is marked with "Other" or with “One or more DD Conditions” and the description section is left blank.
For members with "One or more DD Conditions” checked off please also click on the "Other" and enter in the DD Description of the member to fix this error.
Mbr D/C from ACT. ACT Mbr field must = N
This issue affects billing instance submissions for members who have had a service date within 6 months of ACT expiry date. Until this is fixed by MAPP (by their next release dated for March), the resolution is to answer the BSQ directly into MAPP for the month being impacted by the error (reach out to FCM Help Desk for assistance with this).
