Foothold Care Management generates CMA statements on behalf of the Health Homes based on each HH’s chosen frequency of statements.
More specifically, BHH, CBC, CCC, CCMP and QCCP send statements on a weekly basis and CHN, MSHH, and MSHHSC send statements on a biweekly basis.
Each statement provides a summary of the payments received by the Health Home on behalf of the CMA during the statement period. On certain statements, Health Home administrative fees are also assessed; in these cases, the administrative fee amount is subtracted from the Health Home payment amount to produce a net amount payable from the Health Home to the CMA.
Each statement emailed to CMAs contains a summary of the payments and fees assessed during the most recent statement period. The line-level details associated with that statement can be found directly in Foothold Care Management. To locate these, either copy the link provided in the statement PDF or navigate to https://rma.healthcare/billing/statements in FCM. Once in the appropriate statement summary, users can export the line level details and analyze them as needed in Excel.
Statement Period. Each statement covers either a one-week or a two-week period (dependent on HH). Each statement includes a record of all payments received by the Health Home during the statement period, regardless of the date of service of the claim.
Statement Recipients. Each CMA can indicate in FCM the individual(s) who should receive CMA billing statements via email. To add or remove a FCM user from the statement emails CMA admins can go to Manage -> Users -> (Choose User) -> Edit and then check or uncheck the box that says "Receive billing statement emails." Administrative Fees. Administrative fees are assessed on the first CMA statement in the month following the date of service. For example, administrative fees for May 2019 dates of service are assessed on the CMA statement generated on June 6, 2019. For more information, please see the article Administrative Fees.
