Foothold Care Management generates and transmits claims on behalf of the Health Home, based on the information provided in billing support, on a weekly schedule (every Tuesday).
Billing for Previous Dates of Service. During each claim submission, FCM submits all eligible claims for the current month as well as newly eligible claims from previous dates of service. Each Health Home maintains their own policy defining how long after the original date of service an associated claim may be generated. Note that the date of service on all Health Home claims is the first of the month, not the date that a core service was provided during the month.
