Users enrolling both adult and children members into Health Home with a 12/1/23 Segment Start Date will need to submit their Initial Appropriateness criteria within 30 days of their signed consent.

Users will be able to choose the applicable appropriateness criteria from a dropdown menu. Depending on the option chosen, the system may prompt additional information. For example, if a User chooses the appropriateness criteria highlighted in the screenshot below, a text box will appear for the User to include some additional information:
If this required field is left blank, the system will return an error and will not save until the text box has been filled out.
