The Care Plan now has a dedicated space to document a member's Strengths, Barriers, and Risk Factors. This can be found on the top left corner of the Care Plan's sidebar, as shown in the screenshot below:
To add information here, Users can click the little pencil icon, which will prompt a window to pop up:
Highlighted in yellow is a confirmation of the member's CMA and HH. In this example, clicking on the blue hyperlink will generate a text box for the User to enter in their member's strengths. If the member has more than one, Users can continue to click the hyperlink to add more.
When the User saves the information, the sidebar of the Care Plan will look like this:
If a strength, barrier, or risk factor is no longer applicable to the member, Users have the ability to reflect this by unchecking the Active box.
This will be removed from the active Care Plan view on the sidebar, but doesn't remove it from the record. Should it become applicable in the future for the member, Users can click the pencil icon again to edit and check the Active box to re-populate on the sidebar.
Should a deletion be necessary instead of a deactivation of a strength, barrier, or risk factor, this is possible by clicking the delete icon when editing. This action will gray out field, but will not be deleted until the User clicks on the blue Save button. If the User no longer wants to delete the strength, barrier, or risk factor, clicking on the reverse arrow will undo the action and re-activate the field.
If a User creates a new Care Plan under the same HH/CMA for the member, these will be carried over into that new Care Plan.
Future enhancements will include the Strength, Barriers, and Risk Factors be added to the member's PDF print out of the Care Plan.
