Users have the ability to document a member's Clinical Verification for Enrollment or SED Verification to comply with their Health Homes' policies. This can be done by uploading these documents in a member's chart. Users can find this on a member's Document Tab and upload a document under the Medical category:
Once here, the User can either scroll to the document type they are looking for or type in "Verification" and see the options available to them under the drop down menu for Type. Users can upload a Clinical Verification for Enrollment and/or SED Verification Documentation:
Users can choose their Document Type, enter in a description of the document, and finally the date that this document was created. Don't forget to upload the document itself! Once all these fields have been entered, click on the blue Upload Document button and this will be saved to the member's chart:
The system should redirect the User back to the member's Documents tab and see a confirmation at the top of the page of a successful upload as well as the document housed under the Medical category:
