Having trouble finding a member chart who was previously enrolled under your CMA or Health Home? Oftentimes, this is due to the member chart being re-assigned to another CMA and/or Health Home. But, don't fret, Users do have a way to access the historical information of that member's chart during the time of the member's enrollment by using the Transferred Patients Report. To check for this, follow the steps below.
- Click on Manage --> Reports --> Historical Data --> Transferred Patients
- Enter the member's CIN, or a list of CINs, and Filter.
- You should see a historical version of the member's chart which includes access to historical information such as applicable documents, encounters, assessments, care plans, gaps in care, transitions of care, segments, and billing:
