Member charts will be getting a new look with the addition of the Transitions of Care tab and the removal of the Clinical Notifications tab.
Users will be able to merge alerts to keep them together as one event as we understand that members may have multiple notifications relating to the same clinical event.
Below is an example of a member who had an ER admission and discharge on the same day. Users can check the boxes and click on Merge Clinical Events to create one event.
To ensure that this is what the User wants, a warning will pop up:
There will be an option to set the status of this merged clinical event before the merge is confirmed.
After clicking the save option, the tab will update its view to reflect the merged alerts into an event:
Users can update the status at anytime by using the drop down menu option.
Clicking on the Details hyperlink to the right of the event will take users to a new window. This window has the ability to collapse the details of the specific alerts, similar to its view while under the Clinical Notifications tab.
Within the same window of viewing the Clinical Event Details, conveniently, Users can link encounters to the event.
By clicking on the Create New Encounter button, the system will navigate to a new Encounter screen, where the User can document the encounter for the clinical event.
When creating an encounter this way, Users can note a new tab to the right of encounter note with the ability to link to the Clinical Event. By default, the event itself will be shaded a gray color to reflect the linkage.
After saving the encounter, Users can check the Transitions of Care Tab and see that the encounter is in fact, linked to the event:
Clicking on the details of the event will show any history of linked encounters, like below:
Users will be able to view reports on member Clinical Events using the new CM Tool found in the Manage button of the app:
Users will come to this window, where they will be able to filter the data by a number of different categories. The data will also be exportable as a CSV file for additional review.
After filtering, the members will generate below and Users will be able to see a list of clinical alerts and any merged events for their members.
