Users have the ability to create a new member chart in FCM if no record of the member exists. To check that a chart truly does not exist for the member with a specific Medicaid CIN, Users can follow the below steps:
On the Patient Index Screen, click on the New Patient button on the top right corner:
Enter in the Medicaid CIN and click the Enter key. If the member chart exists in FCM, a message will pop up in red telling a User that this record exists and under which organizations.
If you are a User at a CMA, reach out to your Health Home contact for help with chart access.
If the Medicaid CIN does not exist in FCM, you will see that the system prompts you for the minimum data points needed to create a new chart. These include:
- First Name
- Last Name
- DOB
- Gender
- Medicaid Gender
A User can complete the rest of the demographic information, being as specific as possible and ensuring they choose the correct Health Home, Care Management Agency, and Care Manager to which this member will be assigned.
Once done, click on the blue Create Patient button:
The screen will direct you to the new member's chart Overview:
